All you have to do is delete the PDFMaker.dot template
from Word's startup folder. The path to the file is
usually C:\Program Files\Microsoft Office\Office\Startup.
If this path doesn't exist, search for the template name
and delete it.

Alternatively, if you don't want to delete the template,
go to Word's Tools | Templates and Addins menu. You should
see one or more templates on the list. Just uncheck the
box before the PDFMaker.dot template. You can easily re-
enable by re-checking the box.

Deleting the add-in also removes the ability of Acrobat to retain elements
of the document when converted to PDF and is not a sensible approach. The
method in the link Greg provided automates the temporary removal of the
toolbar without removing the add-in.
